Comprehensive Care

At Hope Harbor Adult Foster Home, we provide a comprehensive range of services consistent with Oregon Administrative Rules (OAR), delivered through a person-centered, clinically informed approach that honors each resident’s dignity, preferences, and evolving care needs. Our home is supported by experienced caregivers and a nurse available 24/7, ensuring continuous oversight, timely clinical judgment, and seamless coordination of care. This added level of professional nursing presence offers families confidence that changes in condition are promptly recognized and addressed.

Personal Care & Activities Of Daily Living

We offer full assistance with all ADLs, including bathing, grooming, dressing, toileting, continence care, transfers, ambulation, and mobility support. Care is provided using proper body mechanics, fall-prevention strategies, and skin-integrity monitoring, while encouraging resident participation to promote independence and functional ability.

Medication Management & Health Monitoring

Our caregivers provide medication administration and assistance in strict accordance with OAR, physician orders, and individualized care plans. Services include medication set-up, administration, monitoring for side effects or adverse reactions, documentation, and timely communication with healthcare providers. Ongoing observation of vital signs, pain levels, hydration status, and overall health supports early intervention and continuity of care.

Dementia, Stroke, and Neurological Support

Hope Harbor is equipped to care for residents living with Alzheimer’s disease and related dementias, as well as those recovering from stroke, Parkinson’s disease, and other neurological conditions. Care includes cueing, redirection, validation techniques, mobility support, assistance with communication challenges, and monitoring for cognitive or neurological changes—all within a calm, structured, and reassuring environment.

Chronic Conditions & Specialized Support

We support residents with a wide range of age-related and chronic conditions, including diabetes, cardiovascular disease, arthritis, mobility impairments, and other elderly disabilities as applicable under OAR. Care is coordinated with medical professionals, therapists, and families to ensure services remain aligned with each resident’s goals and clinical needs.

Nutritious Meals & Personalized Nutrition

Residents enjoy wholesome, home-prepared meals thoughtfully planned to meet individual dietary needs, medical recommendations, cultural preferences, and personal tastes. We accommodate special diets such as diabetic, heart-healthy, soft, or modified-texture meals, while monitoring nutritional intake and hydration to support overall wellness.

Meaningful, Relatable Activities & Engagement

We offer daily activities designed to promote cognitive stimulation, social connection, and independence, including gentle exercise, music, conversation, creative pursuits, and individualized interests. Activities are adapted to each resident’s abilities and preferences, supporting emotional well-being and quality of life. At Hope Harbor, services extend beyond meeting needs—they reflect a deep commitment to compassionate, knowledgeable, and individualized care. With experienced staff, continuous nursing support, and a warm, home-like setting, families can trust that their loved ones are receiving attentive care that balances medical insight with genuine human connection.
